Wegovy: cost, evidence, and the honest comparison
Semaglutide mimics GLP-1, a hormone the gut releases after eating. It slows how fast the stomach empties and acts on appetite centres in the brain, which is why most people report eating less without deliberately trying to.
What it's approved for
Chronic weight management in adults and adolescents 12+ with obesity, or overweight with at least one weight-related condition; also approved to reduce major cardiovascular events in adults with established cardiovascular disease and overweight or obesity.
Forms and doses: Once-weekly injection pen (0.25–2.4 mg, plus a 7.2 mg high dose) and, since January 2026, a once-daily oral tablet.
What the research actually shows
In the STEP 1 trial (NEJM, 2021), participants on semaglutide 2.4 mg lost roughly 15% of body weight on average over 68 weeks versus about 2.4% on placebo. The DXA substudy also showed about 10% of lean mass was lost alongside roughly 19% of fat mass — the reason our diet guide is built around protein and resistance training.

Side effects, stated plainly
The most commonly reported effects in trials were gastrointestinal — nausea, diarrhoea, vomiting, constipation — usually worst during dose escalation. Serious warnings on the label include thyroid C-cell tumours in rodents, pancreatitis, and gallbladder disease. Full risk information belongs with your prescriber and the label.

Wegovy is where the 2026 price war landed hardest: the manufacturer's own pharmacy sells it for $149–$349 a month depending on form and dose, while telehealth brands resell the same medication at the same med price plus a required membership of about $149 a month.

Do I keep the weight off after stopping?
Trials that withdrew the medication showed most participants regained a substantial share of the weight lost, which is why habit and nutrition work during treatment matters so much. That is a conversation for your prescriber.
