The $149 membership tax on your GLP-1

The same Wegovy pill is $149 a month from the manufacturer and about twice that from the biggest telehealth brands. The difference isn't the medication. It's a required membership fee most shoppers don't see until checkout.

In 2026 the GLP-1 price war finally produced honest sticker prices: Novo Nordisk sells Wegovy directly for $149–$349 a month depending on form and dose, and Eli Lilly sells Zepbound vials for $299–$449. Those are real cash prices from the manufacturers' own pharmacies.

The telehealth brands buy from the same manufacturers — since the compounding crackdown, Hims & Hers, Ro, and LifeMD all sell brand medication at prices that match the manufacturer programs. Their pricing pages look identical to NovoCare's. Then, at checkout, a second line item appears: a required membership. Roughly $149 a month, billed separately from the medication, every month you're a patient.

What the $149 actually buys

To be fair to the telehealth brands: the fee is not pure air. The manufacturer pharmacies have a catch — NovoCare and LillyDirect fill prescriptions; they don't write them. You need a prescriber. The membership fee buys you that prescriber, plus refill management, dose adjustments, side-effect support, and (at LifeMD) insurance navigation and labs. If you don't have a doctor you can ask, or your doctor won't prescribe, that service has real value.

The honest framing: you're paying roughly $1,800 a year for a prescription relationship. Sometimes that's rational. Often it isn't.

How to not pay it

  1. Ask your own doctor first. A prescription from any clinician lets you buy at NovoCare or LillyDirect directly and skip the membership entirely.
  2. Use a pass-through telehealth visit. Sesame, for example, sells one-off visits and passes NovoCare pricing through without a required monthly membership (an optional subscription exists — read the checkout).
  3. If you stay with a membership brand, prepay. Ro cuts its fee from $149 to $74 a month on a 12-month prepay — that's $900 a year back for the same service.
  4. Check insurance before paying cash at all. Commercial plans can beat every number on this page, and Medicare Part D's GLP-1 Bridge has offered a flat $50/month copay on Wegovy, Foundayo, and Zepbound since July 2026.
