Daily or as-needed? The cost half of the question

Which regimen suits you is a clinical question and not one for a price table. Which is cheaper is arithmetic, and the answer is more surprising than it looks: it barely depends on how often you need it.

The crossover is where the two regimens cost the same. Divide a daily plan's monthly price by the same seller's as-needed price per dose, and you get the number of doses per month at which it stops mattering. Below it, as-needed is cheaper. Above it, daily is.

At the cash-pharmacy floor that crossover is 51 doses a month — more than once a day. At telehealth subscription prices it drops to single digits.

Every figure is authored from a published row rather than parsed out of a price label, because several labels carry a range or a strength ladder and picking a number silently would decide the comparison by accident. The basis column says which published figure each one uses.

What the table actually shows

The crossover is a fact about the seller, not about you. It ranges from 8.9 doses a month to 51, and that spread has nothing to do with anybody's frequency — it is entirely the difference between paying pharmacy prices and paying subscription prices.

The comparison nobody sets up. At the cash floor, a whole month of daily tadalafil costs $11.22. At the most expensive channel we track, two as-needed doses cost $0.44.

Which is to say: the regimen you choose moves your cost by a factor of a few. The channel you choose moves it by a factor of tens. If you are optimising the wrong one, you are optimising the small one.

What this does not tell you

  • These are different medicines, not two settings of one. Daily plans here are tadalafil; as-needed comparisons are mostly sildenafil. They differ in duration and in how they are taken.
  • Daily tadalafil has uses beyond this one. It is prescribed for other conditions too, which can make the regimen the right choice for reasons that have nothing to do with cost.
  • Frequency is not a purchasing decision. How often a medicine is appropriate for you is a clinical matter, and the arithmetic above should follow that decision rather than lead it.
  • Chewables and troches are priced per dose at a premium over the same molecule as a tablet — that is a formulation choice, and the composition tracker covers what is actually in them.
