Topical or oral finasteride?

This is the one hair-loss decision where the expensive option has a real argument behind it. Here is what the head-to-head trial actually found, and what the price difference is buying.

The short answer. In the trial that compared them head to head, hair counts came out numerically similar — 20.2 hairs with topical against 21.1 with oral at 24 weeks. What differed was systemic exposure: maximum plasma finasteride was over a hundred times lower with topical, and serum DHT fell 34.6% rather than 55.6%.

So the extra money is not buying more hair. It is buying less of the drug reaching the rest of you.

The caveat that decides how far this evidence travels. The trial tested one specific topical spray formulation. Most topical products sold by telehealth brands are compounded combinations — usually finasteride with minoxidil, sometimes with other actives — at strengths the trial did not test. The evidence supports the general proposition that topical finasteride can work with far lower systemic exposure. It does not validate any particular compounded gel, and no compounded gel has a trial of its own.

The trade, stated plainly#

Topical runs $50 to $139 a month against $5 to $20 for oral — roughly 10 to 28 times. Across a year that is a few hundred dollars against a few thousand.

If systemic exposure is what worries you, the trial gives that concern a real number rather than a vibe: a hundredfold lower peak plasma level and roughly half the DHT reduction. That is what you are paying for.
If it is not what worries you, the same trial says you are paying ten to twenty-eight times more for a numerically similar hair count.
Most topical products bundle minoxidil, which has its own evidence and is also available separately and cheaply. Part of what looks like a topical-finasteride premium is a combination-product premium.
The most expensive rows are not finasteride at all. The dearest product we track is a topical dutasteride gel, a different molecule with a different evidence base.

What we are not doing here. Finasteride's side-effect profile — including the sexual side effects that drive most interest in topical formulations — is a genuine clinical topic and not one a price comparison should adjudicate. The trial finding above is reported as published; whether it should change what you take is a conversation with a prescriber who knows your history.
