Tadalafil: cost, evidence, and the honest comparison
Tadalafil blocks the PDE5 enzyme, which allows blood vessels in the penis to relax and fill in response to arousal. It does not create arousal; it removes a hydraulic obstacle to it.
What it's approved for#
Erectile dysfunction, benign prostatic hyperplasia symptoms, and — at specific doses under a different brand — pulmonary arterial hypertension.

Tadalafil's long half-life is what distinguishes it from sildenafil — the reason it is sometimes taken as a low daily dose rather than before activity, and why it acquired the 'weekend' nickname.

Side effects, stated plainly#
Headache, flushing, indigestion, back and muscle aches, and nasal congestion are the commonly reported effects. Critically, PDE5 inhibitors are dangerous with nitrates — a hard contraindication that belongs in a clinician conversation, not a checkout flow.
What it costs#
Generic tadalafil is one of the widest price spreads we track: roughly $0.20 a tablet at the cash floor versus $8 to $12 a dose at some subscription services, and daily tadalafil ranges from about $11 a month to over $1,000 depending entirely on which door you walk through.
