Hair beyond the prescription
Finasteride and minoxidil have the strongest evidence and the cheapest generics. But nutrition status, stress physiology, and a couple of natural options have real literature behind them — and the supplement aisle has a lot that doesn't. The graded version:
First, sort the type of loss
Pattern loss (androgenetic alopecia) is hormonal and gradual — that's the finasteride/minoxidil territory. Diffuse shedding a few months after a stressor (illness, crash diet, surgery, postpartum) is usually telogen effluvium, which typically resolves when the trigger does. Nutrition matters most in the second category and in anyone actually deficient.
The nutrition layer — test, don't guess
- Iron/ferritin. Iron deficiency is the classic nutritional contributor to shedding, especially in women; the JAAD review by Trost et al. discusses ferritin testing in the hair-loss workup while noting the evidence stops short of recommending universal screening. Translation: worth checking with your clinician if shedding is diffuse — and correcting a real deficiency helps, while iron supplementation without deficiency does nothing good.
- Vitamin D and zinc. Reviews of nutrition and hair (Almohanna et al., Dermatology and Therapy) find deficiency states associated with several hair-loss types — and, consistently, that supplementing only helps the deficient. The play is a blood panel, not a gummy subscription.
- Protein and crash diets. Hair is protein; aggressive caloric restriction is a known shedding trigger. Anyone losing weight fast — including on GLP-1s — should read this next to our protein playbook.
The natural options, graded
- Rosemary oil — the one with the famous head-to-head. A randomized trial (Panahi et al., Skinmed, 2015) compared rosemary oil to minoxidil 2% over six months and found comparable hair-count improvement (with less scalp itching). Honest caveats: one trial, versus the weaker 2% minoxidil, modest effect. Grade: the most defensible natural topical, expectations modest.
- Saw palmetto — mechanism-adjacent, evidence thin. It plausibly nudges the same DHT pathway as finasteride, and a systematic review of the small trials finds modest improvements — clearly weaker than finasteride's evidence base. Grade: something, but the generic pill it imitates costs $5–11 a month with far stronger data.
- Biotin — the best-marketed non-answer. Deficiency is rare; evidence for supplementation in non-deficient people is essentially absent, and high doses can interfere with lab tests (including cardiac markers) — a documented FDA safety communication. Grade: skip unless deficient.
- Stress management, unsexy but mechanistic. Telogen effluvium is stress physiology by definition. Sleep and stress load are upstream of more shedding than the supplement aisle admits.
